Who This Texas POA Package Is For

The Texas Power of Attorney Documents package is designed for an individual or couple who already has a will or trust but needs current financial and medical incapacity documents. These documents name the people who may help during your lifetime; they do not replace an inheritance plan.

What’s Included

• Medical Power of Attorney — Lets you name an agent to make healthcare decisions if you cannot make them.
• HIPAA Authorization — Authorizes the people you select to receive medical information as directed.
• Directive to Physicians, Family, or Surrogates — Records your directions concerning life-sustaining treatment in the circumstances covered by the document.

Individual and Couple Options

Choose the option for one individual or a married couple. Each person completes the choices needed for that person’s documents and follows the included review and signing instructions.

What This Package Does Not Include

This package does not include a Last Will and Testament or revocable living trust. If you need inheritance planning too, compare the complete Texas will and trust plans. If you are a single Texan age 18–26, review the Young Adult Power of Attorney Plan, which also includes a simple will.
